Concussion Awareness

Under Rowan's Law, before any player can be registered with Turtle Club Baseball and Softball, LaSalle Athletics Softball or LaSalle Titans Baseball, and Baseball Ontario or the Provincial Women's Softball Association of Ontario ("PWSA"), the player, and the parent or legal guardian of the player if the player is under 18 years of age, must review the concussion awareness resources provided by the Province of Ontario, and must review the concussion codes of conduct of the organizations they are registering with. I/we hereby confirm that the player being registered to participate in baseball and/or softball activities with Turtle Club Baseball and Softball, LaSalle Athletics Softball and/or LaSalle Titans Baseball, and Baseball Ontario or the PWSA (the "Organizations"), and the parent(s) and/or legal guardian(s) of the player being registered if the player is under 18 years of age, have reviewed the concussion education awareness resources provided by the Province of Ontario as referenced above and have reviewed the codes of conduct as referenced above. I/we further acknowledge and understand the nature and risk of concussion and head injury to athletes, including the risks of continuing to play after a concussion or head injury is suspected.

I/we authorize the Organizations, and their respective staff, associates, assistants and subcontractors, to use photographs, video and other media images provided for any Organizations' promotional, educational or other pertinent uses. I/we also agree to the Organizations using any images of the player being registered gathered by photographers and/or videographers supplying images to the Organizations. These images may include, but are not exclusive to, club, coach, athlete, parent and volunteer submissions by the Organizations and by other member associations of Baseball Ontario and the PWSA. I/we authorize the Organizations to permit the use and display of photographs and recordings of the registered player in any Organizations' publications, multimedia productions, including video and web usage, display or advertisement. I/we agree that the Organizations may use the name, likeness or information supplied by me or the registered player. I/we release and forever discharge the Organizations, and their respective officers and employees, and all member associations of Baseball Ontario and the PWSA, from any and all claims and demands arising out of or in connection with the use of said photographs and recordings, including, but not limited to, any claims for invasion of privacy or defamation.
